      How to File a Claim

      We understand that accidents are going to occur and we have experts available to walk you through the claims administration process.

      It is extremely critical that all accidents be reported as soon as possible.

      Step 1
      Download and complete the claims submission form.

      Step 2

      Submit form to Landin.

      Please fill out the form legibly and make sure the person completing the report is authorized.  Provide as much information as possible so that an accurate investigation of each claim can be conducted.  This will ensure that an injured employee will receive the benefits to which he/she is entitled.  On the other hand, those who are not eligible to receive benefits will be identified.

      If you do not know the answer to some of the questions or if they do not apply, write either “unknown” or “N/A” in those spaces.  If you have any questions whatsoever, please feel free to call us and we will be glad to assist you.

      IMPORTANT REMINDER:  Do not file the First Report of Injury Form (FROI) with the Virginia Workers’ Comp Commission.

      File with Company Nurse

      If you are enrolled with Company Nurse, you can file a claim through their system.
